Payment
Insurance: I accept all PPO insurances and will submit claims for you. You may contact your insurance company to determine your deductible and percentage of the payment covered. If you are asked, I am an “out-of-network provider in a professional setting.”
I offer a sliding scale when needed. I accept checks, cash, and credit card.
Forms
Prior to the initial intake session, please take the time to read and sign the consent form. We will review it during the initial session. In addition, please complete the appropriate intake form. The HIPPA link provides important information about your privacy rights. The information is for you to keep, the signature page indicates that you have received the information.
You may print the forms and bring signed copies to the session or complete the forms electronically.
